Accuracy of diagnostic tests to detect asymptomatic bacteriuria during pregnancy.

Journal of Obstetrics - Fri, 01/23/2009 - 05:58
CONCLUSION:: A pregnant woman with a positive dipslide test is very likely to have a definitive diagnosis of asymptomatic bacteriuria, whereas a negative result effectively rules out the presence of bacteriuria. Dipsticks that measure nitrites and leukocyte esterase have low sensitivity for use in screening for asymptomatic bacteriuria during gestation. Perinatal outcome of monoamniotic twin pregnancies.

Journal of Obstetrics - Fri, 01/23/2009 - 05:58
CONCLUSION:: The current incidence of perinatal mortality in monoamniotic twins is considerably lower than in previous decades, but it is still high and occurs throughout pregnancy. LEVEL OF EVIDENCE:: III. PMID: 19155906 [PubMed - as supplied by publisher] (Source: Obstetrics and Gynecology)

Risk factors for neurodevelopment impairment in twin-twin transfusion syndrome treated with fetoscopic laser surgery.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
CONCLUSION:: Neurodevelopment impairment in twin- twin transfusion syndrome survivors treated with laser surgery is associated with advanced gestational age at laser surgery, low gestational age at birth, low birth weight, and high Quintero stage. LEVEL OF EVIDENCE:: II. PMID: 19155907 [PubMed - as supplied by publisher] (Source: Obstetrics and Gynecology)

Complication and reoperation rates after apical vaginal prolapse surgical repair: a systematic review.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
CONCLUSION:: The rate of complications requiring reoperation and the total reoperation rate was highest for vaginal mesh kits despite a lower reoperation rate for prolapse recurrence and shorter overall follow-up. PMID: 19155908 [PubMed - in process] (Source: Obstetrics and Gynecology)

Low-dose oral misoprostol for induction of labor: a systematic review.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
CONCLUSION:: Low-dose oral misoprostol solution (20 micrograms) administered every 2 hours seems at least as effective as both vaginal dinoprostone and vaginal misoprostol, with lower rates of cesarean delivery and uterine hyperstimulation, respectively. PMID: 19155909 [PubMed - in process] (Source: Obstetrics and Gynecology)

The accuracy of risk scores in predicting ovarian malignancy: a systematic review.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
CONCLUSION:: Based on our review, the Risk of Malignancy Index should be the prediction model of choice in the preoperative assessment of the adnexal mass. PMID: 19155910 [PubMed - as supplied by publisher] (Source: Obstetrics and Gynecology)

Smaller pieces of the hysterectomy pie: current challenges in resident surgical education.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
Smaller Pieces of the Hysterectomy Pie: Current Challenges in Resident Surgical Education. Obstet Gynecol. 2009 Feb;113(2, Part 1):395-398 Authors: Pulliam SJ, Berkowitz LR Residents in obstetrics and gynecology are increasingly confronted with a wider range of techniques that must be mastered to perform hysterectomy, including abdominal, vaginal, laparoscopic, and robotic approaches. This is accompanied by a decrease in the number of hysterectomies performed annually. Possible solutions to the dilemma created for surgical teaching includes a comprehensive program evaluating surgical competency by establishing numbers needed to achieve competency for specific major procedures. Performing a fetal anatomy scan at the time of first-trimester screening.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
Performing a Fetal Anatomy Scan at the Time of First-Trimester Screening. Obstet Gynecol. 2009 Feb;113(2, Part 1):402-407 Authors: Timor-Tritsch IE, Fuchs KM, Monteagudo A, Dʼalton ME Over the past decade, prenatal diagnosis has shifted rapidly from the second trimester into the first trimester. Although the nuchal-translucency scan may detect a small proportion of fetal structural malformations, fetal anatomy is not routinely assessed until the fetal anatomical survey is performed in the second trimester between 18 and 22 weeks. The recent development of high-frequency transvaginal ultrasound transducers has led to vastly improved ultrasound resolution and improved visualization of fetal anatomy earlier in gestation. Several pilot studies of a first-trimester anatomic su...

Fetal intervention: providing reasonable access to quality care.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
Fetal Intervention: Providing Reasonable Access to Quality Care. Obstet Gynecol. 2009 Feb;113(2, Part 1):408-410 Authors: Moise KJ, Johnson A, Carpenter RJ, Baschat AA, Platt LD Interest in fetal intervention has become widespread in recent years. Laser therapy for the treatment of severe twin-twin transfusion alone has been the subject of more than 100 peer-reviewed articles in the past 3 years. Significant issues have arisen that affect the availability of these new therapies in the United States. Formal training fellowships have yet to be established. Questions as to the ultimate number of treatment centers that are required to provide reasonable access should be addressed. The establishment of research networks to evaluate new fetal therapies through randomized clinical tri...

Medications in pregnancy and lactation: part 2. drugs with minimal or unknown human teratogenic effect.

Journal of Obstetrics - Fri, 01/23/2009 - 05:57
Medications in pregnancy and lactation: part 2. Drugs with minimal or unknown human teratogenic effect. Obstet Gynecol. 2009 Feb;113(2 Pt 1):417-32 Authors: Buhimschi CS, Weiner CP This is the second of a two-part series on the use of medication during pregnancy and lactation. Pregnancy risk factors together with an increased incidence of chronic diseases and the rise in mean maternal age predict an increase in medication use during gestation. However, as highlighted in the first installment of this series, relatively few medications have specifically been tested for safety and efficacy during pregnancy, and, therefore, responses to those inquiries can be uninformed and inaccurate. Whereas the first installment provided new insight into the nature of medications with known huma...
