Discussion. We also indicated that with the purpose of additional future validations, we wanted to provide a fixed classification model. Because the training samples in our article serve as a training set not only for this specific study but also for future validation studies, it was deemed more efficient to combine all samples from the training and test sets to refine the model. On page 1070 of the original article, it is acknowledged that "the final model provides more optimistic results compared with the test group, because it includes the training set... Nevertheless, this final model will be validated in a multicenter validation study." In addition, we used 10-fold cross-validation for the total cohort, which is a well-accepted approach. There are numerous studies using this analysis m...

Depo-provera and skeletal health: a survey of florida obstetrics and gynecologist physicians.

Wed, 10/22/2008 - 09:58
CONCLUSION: Respondents may be writing fewer prescriptions for DMPA, are likely to institute a time limit on said prescription and are likely to order BMD testing, using the black box warning as justification. Continued education is necessary to prevent inappropriate restrictions on DMPA use and the performance and/or prescription of inappropriate tests and medications. Fetal mesenchymal stem cells derived from human umbilical cord sustain primitive characteristics during extensive expansion

Wed, 10/22/2008 - 01:29
Abstract  Stem cells of fetal origin lie between embryonic and adult stem cells in terms of potentiality. Because of the ethical controversy surrounding embryonic stem cells and the relatively inferior quality of adult stem cells, the use of fetal stem cells would be an attractive option in future therapeutic applications. Here, we have investigated primitive characteristics of human umbilical-cord-derived fetal mesenchymal stem cells (UC fMSCs) during extensive expansion. We have successfully isolated and cultured UC fMSCs from all UC samples, but with two early fungal contaminations. UC fMSCs proliferated without significant evidence of morphological changes, and the average cumulative population-doubling level was over 25 for about 3 months. UC fMSCs showed the posi...

Exacerbation of liver cirrhosis in pregnancy: a complex emerging clinical situation

Wed, 10/22/2008 - 01:29
We report a descriptive management of pregnancy in a woman suffering from chronic viral hepatitis B with D and C coinfection exacerbated finally to severe cirrhosis of the liver. Caesarean section was performed giving birth to a viable male infant. Two months after the operation the patient underwent a liver transplantation with good outcome. Management of such severe cases requires a multidisciplinary medical approach. Data regarding cirrhosis caused by chronic viral agents in pregnancy are limited. Coffee intake, variants in genes involved in caffeine metabolism, and the risk of epithelial ovarian cancer

Wed, 10/22/2008 - 01:03
Abstract  We evaluated whether genetic variability, as well as menopausal status, modify the association between coffee intake and risk of ovarian cancer. Risk factor information and biologic specimens from three large epidemiological studies, the Nurses’ Health Study (NHS), NHSII, and the New England based Case-Control Study of ovarian cancer (NECC) were pooled resulting in 1,354 ovarian cancer cases and 1,851 controls for analysis. Odds ratios (ORs) and 95% confidence intervals (CI) were estimated using conditional (NHS/NHSII) and unconditional (NECC) logistic regression. Coffee consumption was not associated with overall risk (OR = 0.99; 95% CI 0.77–1.28); however, there was a suggested increased risk of ovarian cancer among premenopausal women in the NECC o...

House-passed bill would create more primary care residency programs

Wed, 10/22/2008 - 00:00
The House has taken steps to alleviate the growing shortage of primary care physicians by passing a measure that would establish an interest-free loan program for hospitals to start new osteopathic or allopathic residency training programs. The Physician Workforce Enhancement Act of 2008, H.R. 2583, now under consideration by the Senate Health, Education, Labor and Pensions Committee, would make interest-free loans available to hospitals to start new osteopathic or allopathic residencies in one of five medical specialties or some combination of the specialties. The five specialties are family medicine, internal medicine, pediatrics, obstetrics and gynecology, and behavioral or mental health. (Source: AAFP Government and Medicine)

Disease confined within the ovary and smaller amount of ascites are good prognostic factors for survival of patients with squamous cell carcinoma arising from mature cystic teratoma of the ovary: a case series in korea and review of the published reports

Tue, 10/21/2008 - 23:00
Aim: To identify clinical prognostic factors for survival of patients with squamous cell carcinoma (SqCC) arising from mature cystic teratoma (MCT) of the ovary with review of the published reports. Methods: Clinical data of 55 patients with SqCC arising from MCT of the ovary who were reported in the Korean Journal of Obstetrics and Gynecology and the Korean Journal of Gynecologic Oncology and Colposcopy between 1992 and 2006 were reviewed. Clinical factors were analyzed to identify their association with disease-free survival and overall survival (OS), which were evaluated using the Kaplan-Meier analysis with the log-rank test. Clinical prognostic factors were identified using Cox's proportional hazard analysis. Results: Patients with disease confined within the ovary (n = 31) or ascites (S...

Full-term pregnancies after multiple hepatobiliary operations for choledochal cyst: a case report

Tue, 10/21/2008 - 23:00
Choledochal cysts are rare congenital cystic dilatations of the biliary tree. Choledochal cysts in pregnancy are a rare entity and represent a diagnostic and therapeutic challenge. The authors present a case of two full-term pregnancies in a woman who underwent multiple hepatobiliary operations for a choledochal cyst. The patient had two major problems: a type IVa choledochal cyst with atrophy of the left side of the liver and a congenitally dilated pancreatic duct in the head of the gland. She underwent a drainage surgical procedure using a Roux loop of jejunum and a revision surgery in which complete excision of the cyst and hepaticojejunostomy was performed. The surgical management was completed by a left lateral hepatic segmentectomy. During her first pregnancy, she experienced an epis...

Risk factors for cesarean hysterectomy in tertiary center in thailand: a case–control study

Tue, 10/21/2008 - 23:00
Conclusion: Placental adherence, placenta previa and uterine atony were identified as significant risk factors for cesarean hysterectomy. Cesarean section due to placenta previa should be electively scheduled with well-prepared blood components. The obstetrician should provide counsel and obtain detailed informed consent with regard to the possibility of cesarean hysterectomy. (Source: Journal of Obstetrics and Gynaecology Research)

Utero-relaxant effect of pde4-selective inhibitor alone and in simultaneous administration with β2-mimetic on oxytocin-induced contractions in pregnant myometrium

Tue, 10/21/2008 - 23:00
Conclusion: Our data are consistent with previous studies of the enhanced efficiency of the [beta]2-adrenergic agonist, when administered together with the phosphodiesterase 4-inhibitor. Moreover we have shown that rolipram alone has a more profound effect on oxytocin-induced contractions than salbutamol alone. (Source: Journal of Obstetrics and Gynaecology Research)

Why planned attended homebirth should be more widely supported in australia

Tue, 10/21/2008 - 23:00
This article argues that the continuing reluctance on the part of professional and bureaucratic bodies in Australia to provide for and support planned attended homebirth for low-risk women is unfounded according to the research evidence. It also suggests that such lack of support might be encouraging some planned but intentionally unattended homebirths to occur in Australia, particularly as in recent years there appears to have been an increase in popularity in freebirth (or do-it-yourself homebirth). The article calls for RANZCOG and Australian state health departments to support planned attended homebirth for low-risk women in the face of what is now a considerable amount of evidence showing its safety, when compared with unplanned homebirth and hospital birth. The article raises a numbe...
